Summary of Job Function: Under supervision of the department supervisor, the Case Manager is responsible for identifying, coordinating, advocating, linking, and providing crisis services to assigned consumers. Case Managers provide direct and indirect services.

Essential Functions:

  1. Conducts interviews with consumers and their legal guardians, as indicated, to collect information needed to complete a psychosocial assessment.
  2. Works with consumers and other treatment team members following assessment and identification of needs to develop a service/treatment plan within identified timeframe.
  3. Provides supportive intervention services to consumers, assisting them to practice skills learned in therapy and make effective use of resources.
  4. Schedules service/treatment plan reviews at designated timeframes to review consumer progress and amend the plan as needed.
  5. Assesses and reassesses consumers needs by utilizing various screening and assessment tools, reviewing consumers treatment goals, and collaborating with other treatment team members.
  6. Provides targeted case management services to consumers who qualify and need TCM services.
  7. Assures that pertinent consumer information is shared with other treatment team members to promote quality care.
  8. Engages and communicates effectively with consumers and their families/legal guardians, and all team members.
  9. Collaborates and plans effectively to access the needed supports and services for each consumer and family served by working with agencies and organizations in a community.
  10. Schedules appointments and assures all parties have relevant consents and information needed.
  11. Prepares and maintains case records, letters, reports and forms, performs case
  12. follow-up and closing, and performs other administrative tasks within established guidelines/timeframes.
  13. Ability to assess and respond effectively to crisis situations. Utilizes crisis safety planning to prevent crisis situations.
  14. Works with consumers to promote a positive sense of self-worth and independence.
  15. Organizes and conducts support groups for consumers as directed.
  16. Advocates for consumers and their families by attending MDTs, court hearings, IEP/SAT meetings, etc.
  17. Utilizes technology provided to perform job efficiently and effectively.
  18. Documents all activities per policy.
  19. Arranges for consumer transportation to appointments as needed.
  20. Adheres to Prestera Centers productivity standards and attends all required department and supervision meetings.
  21. Attends and remains current on all mandatory agency trainings.
  22. All other duties as assigned.

Job Specifications:

  1. Education, Certification/License, and/or Experience
  • Bachelors Degree in psychology, counseling, social work from a four-year college or university.
  • Valid Drivers License and reliable transportation is required.
  • Experience or prior training working in behavioral health field preferred.

2. Knowledge, Skills, and Abilities required: Must possess the ability to read, write reports, perform basic math (adding, subtracting, multiplying, and dividing), have good computer skills, and have good verbal and written communication skills.
